Job Summary
Join Warde Senior Living as a Full-Time MDS Coordinator for RN or LPN; lead RAI/MDS processes, ensure compliance, and improve resident care in a supportive environment.
Required Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid New Hampshire license
- Minimum 2 years’ experience in long term care including experience with the RAI process and person-centered care planning
- Strong clinical and communication skills
- Extensive knowledge of the RAI process and reimbursement requirements for federal and state
- Highly organized with the ability to multi-task throughout the day
Desired Qualifications
- RAC-CT certification preferred
- Experience with Microsoft PowerPoint, Word, Excel and Outlook
- Experience in developing templates for audit/educational use
